Just discharged from the hospital? What do I do next?
Being hospitalized for any reason is never a pleasant experience. Now that you’re discharged, coordinating your post hospitalization care becomes a priority to fully recover and heal.
You may need physical therapy, follow-up lab work, wound care, medical equipment, medication refills and care coordination with specialists. This can take time away from focusing on your recovery and cause additional stress, especially if you are homebound and unable to follow up with your regular doctor.
You’ll be surprised that many of the post hospital care you need can be done right in the comfort, safety and convenience of your own home.

Post hospitalization follow-up care
We provide primary care to patients discharged from hospitals throughout Palm Beach County and Northern Broward County. We coordinate and order your occupational or physical therapy, follow-up lab draws, provide referrals to specialists, order home care nursing, order durable medical equipment and provide medication management in your home.

Why didn’t my doctor order post hospital home care?
It is possible your doctor is part of an ACO (Accountable Care Organization) model. This means he or she shares in the savings with health insurance company’s or have to pay back a percentage when they over spend on the care of a patient. Home care is generally more expensive for insurers to pay, so doctors contracted to ACO models will try to control the cost of patient care.
As an independent health care provider, we are not tied to an ACO model or part of an HMO. We can provide home care services for patients that meet specific criteria.
